Observed blood on the perch which was broken.
Briefly examined 015's feet and noticed a broken talon on digit 1. The break appeared to only have included the keratin of the talon and is not assumed to be the source of the blood in the cage. The source of the blood was likely from the food.

Bandage and splint were removed.
There was dried blood on the telfa pad between the skin and the splint at the site of the major metacarpal fracture/callus. When the pad was removed there was a small amount of active hemorrhage. The wound was cleaned with a dilute iodine solution. A calcium alginate pad was placed on top of the wound followed by a tegaderm wrap, then foam padding.
The wing was wrapped in cast padding and vetwrap in a figure 8 pattern.

The splint was left off today.

Notes: Laser therapy with non-contact head. Patient was induced with 5% ISO and maintained at 4%. Heart rate was monitored via auscultation and respiration rate was monitored through visualization. The suture site had some discoloration to it, callous is more stable but we are worried that the SAM splint is applying pressure on the callous so we opted to do a figure 8 wrap with no SAM splint. Patient recovered uneventfully. ROM was WNL.

Physical therapy was not done on 015 due time constraints and them not being under anesthesia at the time of laser therapy. ROM of the left wing appeared to be good, the wound on the metacarpal region appears to be static with some mild serosangenous fluid coming from a non healing area of the wound. A strip of triple antibiotic was placed on the wound and was wrapped up with tagaderm. A Sam splint with a figure 8 wrap was placed on top with distraction tabs to help prevent them from tearing the wrap up.

015 anesthetized with ISO (5%) around 4:10 PM and maintained on 3% during PT. Wing-to-body wrap and sam splint removed. Tegadarm bandage was tightly adhered to skin and removed. The wound is healing by second intention (granulation tissue present). The metacarpus is unstable on palpation. A fibrous callous is present. Laser therapy completed (504 J). Passive ROM performed on left wing. Tegaderm + pad + triple antibiotic applied over wound. A Sam splint was placed on the elbow along with a wing to body wrap. HR (ausculation) and RR monitored during procedure every 2-3 minutes. Iso ended around 4:30 PM (total time under anesthsia 20 minutes). Patient recovered appropriately.

Cast padding was placed betweent the wing and body for comfort, and a wing-to-body wrap was placed. HR monitored with auscultation and doppler during procedure. Under anesthesia for 40 minutes. Patient recovered appropriately.

PT: PROM under anesthesia
- 015 was anesthetized with Isoflurane at 5% initially, then maintained at 3% throughout the rest of the procedure. Heart rate and respiratory rate were monitored q 5 minutes and were within normal limits. Was on ISO for 9 minutes.
- Removed bandage. Wound is still partially open (compared with photo from post-op looks about the same). Some edema present, but no signs of infection.
- Normal ROM: 150 degrees (elbow), 180 degrees (carpus)
- Triple antibiotic applied to wound. Tongue depressor splint attached to outside of wing to avoid pressure on wound. Covered with cast padding and vet wrap to stabilize carpus (figure of 8 wrap).
- Plan: continue PT to check ROM (next Monday) + laser therapy

Surgery report

Initial HR: 200; RR: 16
Patient 015 preoxygenated then went under anesthesia with ISO
at 11:14 AM. HR/RR were monitored every 5 minutes. Cleaned left
carpus with diluted betadine and saline solution. Debrided away
dried blood and feathers from wound edge. A 1cm wound at the
fracture site on the ventral L metacarpal area with bone visible
was found.  The wound was debrided and flushed with 60 mL of
warm LRS.  The skin edges were undermined to reduce tension,
but the wound could not quite be fully cloosed. Two horizontal
mattress sutures (4-0 PDS) were placed. A small amount of Manuka
honey was applied over the wound and then covered with a Tegaderm+pad
bandage. Cast padding was placed betweent he wing and body for
comfort, and a wing-to-body wrap was placed. While under anesthesia
we drew blood from the right jugular for intake lab work. Patient
awake from procedure at 12:16 PM and was put back in cage with
towel ring support. Sutures to be removed in 10-14 days.

015 was anesthetized with iso via mask for radiographs. The patient's heartrate and respiratory rate were take throughout the procedure until the patient was awake. Radiographs were taken to confirm suspicion of the metacarpal fracture 015 presented with. A left wing lateral and cranial/caudal wing view were captured. Upon radiographs, the major metacarpal showed a comminuted fracture, and the minor metacarpal showed a segmental fracture. The major metacarpal fracture only measure ~1.32 cm from the most distal portion. This being said, external fixation would not be a good option due to pin space. A metacarpal splint with recheck of radiographs in 4-6 weeks was chosen.

Upon intake physical examination of the left wing of 015, the wing was heavily edematous, along with skin bruising indicative of inflammation. An additional physical examination was done today. The wing wrap was removed. The wing showed less edema and minimal bruising compared to intake. Overall the wing has visually improved. A figure eight wing wrap was then put on the left wing.

Plan: 1. Radiograph left wing
2. Medications: meloxicam 1 mg/kg, buprenorphine 0.1 mg/kg

Diagnosis: 1. Metacarpal fracture on left wing with edema
Assessment: Based on history, there is an aparent metacrpal fracture as stated by Alford Avenue Veterinary Hospital in Birmingham, AL. After the wing wrap was removed, the fracture was palpated and edema was noted near the fracture sight.This area needs to monitored for devitalization of tissue. There was dried blood around nares that could either be from a past feeding or from suspected trauma.
Plan:
1. Call Alford Avenue Veterinary Hospital in Birmingham, AL on Monday for radiographs of fracture
2. Submit HPAI test
3. Start meloxicam - adminstered 0.11ml of injectable meloxicam (5mg/ml) to the right of the keel bone
4. Adminstered by Dr. Oster - Simbadol 0.03ml for pain control
5. Continue bandaging in a left wing and monitoring left wing every few days (1-2 days) for signs of devitalized tissue around sight of edema
6. Monitor food intake to ensure nutritional needs are met
Prognosis: Fair
